Calverton Rec Council (CRC), Audio, Video and Photographic Release Agreement
By participating in Calverton Recreational Council (hereafter refered to as CRC) sports programs you agree to the following statements regarding you or your childs(hereafter refered to as Participant):
I hereby give CRC, its legal representatives and assigns, those for whom it is acting, and those acting with its permission or its employees, the right and permission to copyright and/or use and reuse, and/or publish and republish, and/or produce and reproduce, and/or broadcast and rebroadcast photographic picture or portraits of Participant, videotapes of Participant, and/or audio tapes of Participant's voice, made through these or any other media without restriction for any legal purpose whatsoever, including any printed matter in conjunction therewith.
Participant hereby waives any right to compensation of any kind, as well as any right to inspect or aprove the finished photograph, video, or audiotape, or advertising copy, or printed matter that may be used in conjunction therewith, or to the eventual use that it might be applied.
Participant hereby release, discharge, and agree to hold harmless CRC, its representatives, assigns, employees, or any person(s), corporation(s), acting under its permission or authority, or any person(s), corporation(s), for whom it might be acting, including any firm publishing, producting, distributing, and/or broadcasting the finished product, in whole or in part, from and against any liability as a result of distortion, blutting or alteration, optical illusion, or in composite form, that may occur or be produced in the talking, processing, editing, or reproduction of the finished produced, its publication, distribution, or broadcast.
